the following information so that we can respond more quickly:
- Your name (first and last as it appears on the account)
- Daytime telephone number
- Evening telephone number
- Best time to call
- Product or service (i.e., credit disability insurance, accidental
death, warranty, etc.)
- Benefit activation number (if known)
- Certificate/policy number
- Account/loan number
- Creditor/bank/loan company (if applicable)
- Retailer or utility (if applicable)
- Description of problem or concern
